Building Pathways to Stability

AGAPE HOLMES INCORPORATION is working to develop programs that support individuals and families experiencing housing instability and other significant life challenges.

Our goal is to provide compassionate, person-centered services that help participants move toward safe housing, self-sufficiency, and long-term stability.

Transitional Housing

We are developing a transitional housing program designed to provide temporary, supportive housing for eligible participants while they work toward permanent housing.

The program is intended to combine safe housing with individualized support and community resource connections.

Case Management

Participants will work with staff to develop individualized plans based on their goals, strengths, and needs.

Case management may include:

  • Goal setting

  • Service coordination

  • Resource referrals

  • Progress reviews

  • Housing planning
